Helpful information to have ready
The form asks for contact and emergency-contact information along with recovery, health, legal, and program-related information. Complete it somewhere you feel comfortable answering private questions. Do not send sensitive medical or legal documents by ordinary email unless the organization specifically tells you how to share them securely.
